Delta Dental of Arizona warns patients of data breach

Delta Dental of Arizona has notified patients of a July phishing scheme that could have exposed their protected health information, according to a Nov. 8 news release

On Jul. 8, a Delta Dental of Arizona employee fell victim to an email phishing scheme that allowed an unauthorized individual access the employee's email account. Delta Dental said there is no evidence that patient information in the email account has been misused.The company also couldn't confirm whether any information was actually accessed by the unauthorized individual.

Member data from the email account that may have been exposed included names, addresses, dates of birth, Social Security numbers, driver's license numbers, passport numbers, financial account information, dental/treatment information, dental insurance information and digital signatures.

Delta Dental of Arizona is encouraging potentially impacted individuals to review account statements and monitor credit reports for suspicious activity.
