The Smiles By Designs building was hit in the morning of July 15 in a one-vehicle accident after the truck malfunctioned, according to the report.
The practice was not open when the crash occurred and there were no injuries reported.
Employees from the practice will contact patients when they have more information about the situation, the report said.
