The Washington Dental Quality Assurance Commission has charged a Kitsap County dentist with unprofessional conduct.
Hamidreza Barakhshan, DDS, allegedly failed to create adequate treatment records in response to a patient complaint, according to a Nov. 22 news release from the Washington State Department of Health.
Dr. Barakhshan has a practice in Bremerton, Wash.
