Washington dentist has license restricted for 'substandard care'

The Washington Dental Quality Assurance Commission and Shawn Keller, DDS, agreed to restrictions of his dental license.

The commission claims that between 2018 and 2020, Dr. Keller provided substandard dental care to multiple patients and was unable to produce adequate patient records to investigators, according to a Jan. 3 news release from the Washington State Department of Health.

During the restriction, which will last at least two years, Dr. Keller must limit his implants, bridges and crowns practice or have a pre-approved proctor.

In addition to the license restriction, Dr. Keller has to submit patient ledgers of previous charges, allow investigations of patient records, comply with infection control inspections, complete a professional ethics course, take a 22-hour dental ethics course and pass a jurisprudence exam.

Dr. Keller also has to pay a $5,000 fine and reimburse the dental commission $10,000, the release said.
