In 2018, Dr. Endicott was arrested and convicted in Arizona for failing to register as a sex offender, and did not disclose the arrest to the Arizona Board of Dental Examiners upon renewal of his license in 2018, according to a Feb. 22 order from the licensing division.
Dr. Endicott performed a dental procedure on a patient in 2021 that resulted in the death of the patient, which was reported to the Arizona Board of Dental Examiners.
The Arizona dental board and Dr. Endicott entered into an agreement prohibiting him from practicing in Arizona, which he did not report to the Utah licensing division.
The division alleged Dr. Endicott engaged in unprofessional conduct by not notifying the division of the revocation of his license by the Arizona board.
Dr. Endicott failed to file a written response to the Utah licensing division, which constitutes a basis for entry of default against Dr. Endicott.
As a result of the decision, Dr. Endicott is barred from practicing as a dentist with or without a local anesthesia permit and cannot administer or prescribe controlled substances in Utah, the report said.
