Hawaii Family Dental, a 12-office practice, recently disclosed a data security incident in which an unauthorized individual used a compromised account to access patient information.
The practice said it immediately secured its network and launched an investigation after discovering the breach July 20. A total of 45,853 individuals were affected by the breach, according to the HHS Department’s Office for Civil Rights Breach Portal.
Information that may have been accessed in the breach includes names, dates of birth, phone numbers, email addresses, mailing addresses, dental insurance information and portions of medical and dental treatment records. Social Security numbers and financial account information were not involved, according to a notice on the practice’s website.
The practice said it has no evidence that any information has been misused.
