2. Since being restricted from practicing endodontics, Dr. Lay was required to inform his patients of other endodontists and transfer patients’ records at no costs to them
3. Dr. Lay was ordered to pay a $5,000 civil penalty. After completing the endodontics course, his license will continue to be on prohibition for five years.
4. Dr. Lay’s dental license will expire Aug. 31, 2020.
