The practice said it became aware in May that its computer systems were affected by a data security incident, which the practice believes began in April. The practice determined the incident resulted in the unauthorized acquisition of certain files, according to a notice on the practice’s website.

The information affected by the incident may include identifying patient information, contact information, financial information, insurance information and diagnosis and treatment information.
