360 Dental, a dental practice in Philadelphia, experienced a computer security incident affecting 11,273 individuals.
On Nov. 16, 2025, the practice identified an authorized party had accessed its internal computer system and locked certain files, which may have exposed information stored on the server, according to a Dec. 27 notice from 360 Dental.
Personal information that may have been involved includes patient names, dates of birth, addresses, telephone numbers, emails, patient account or chart numbers, dental records and Social Security numbers.
Following the breach, the practice replaced affected computers, rebuilt the server, updated software and implemented additional security measures.
The practice submitted the incident to the HHS Department’s Office for Civil Rights Breach Portal Jan. 15.
