Four things to know:
1. The former South Ogden (Utah) Dental office manager, Nikki Lee Martinez, was charged with 25 counts of forgery, obstructing justice, unlawful use of financial transaction and theft by deception.
2. She was fired in October after blank checks were found in her vehicle. According to police, she applied for fraudulent loans, practiced patient signatures and issued $13,700 worth of checks to an individual not associated with the practice.
3. Ms. Martinez also allegedly used the practice’s credit card to pay for Netflix, car payments and groceries, among other expenses.
4. An indictment was filed against Ms. Martinez in December after she demanded payment from a patient be deposited directly into a bank account.
