Kirkland, Wash.-based Totem Lake Family Dentistry experienced a data security incident that may have impacted patient health information.
The practice detected unauthorized access to an employee email account on or about June 2, 2025, according to a notice on its website. An investigation determined that some of the practice’s files, including those that contained protected health information, may have been accessed by an unauthorized individual between May 28, 2025, and June 2, 2025.
The information that may have been accessed in the breach includes names, dates of birth, Social Security numbers, medical records, treatment plans, patient numbers, prescriptions and health insurance information.
More than 3,400 individuals were affected by the breach, according to the HHS Department’s Office for Civil Rights Breach Portal.
The practice said it currently has no evidence that any information has been misused as a result of the cybersecurity incident.
