Charles Pan, DDS, has been accused of operating his practice with inadequate patient records in addition to infection control standard violations. He has been under review for infection control issues since 2015 when the health department ruled Dr. Pan would need to undergo periodic audits.
Dr. Pan has 20 days to respond to the health department’s charges. He is unable to practice dentistry until the charges are resolved.
