Heartland Dental rolls out care coordination platform

Effingham, Ill.-based Heartland Dental launched Conuity Care Coordination to better connect general dentists, specialty providers and patients.

The platform combines technology and care coordination services to manage specialty dental care across referral, scheduling, treatment and follow-up. Currently, only about 15% to 30% of patients referred for specialty care follow through on the recommendation, according to a Sept. 3 news release from Heartland Dental. 

Conuity Care Coordination connects each step for the patient, from initial general dentistry visit through specialty care treatment. 

“At Heartland Dental, we are committed to investing in tools and capabilities that help supported doctors deliver an exceptional patient experience,” Rick Workman, DMD, founder and executive chairman of Heartland Dental, said in the release. “Conuity Care Coordination reflects that commitment by helping connect providers, simplify collaboration and support more seamless care journeys for patients. It is another example of how we continue to evolve our doctor-led culture and support model to better serve supported doctors and the communities they care for.”
