Dental insurance mix-up costs woman $11K: 5 things to know

A filing mix-up between a Chicago woman's dental practice and her insurance company cost her $11,000, CBS Chicago reported Dec. 5.

Five things to know:

1. Sheila Tucker paid her dental practice, Chicago Dental Dream Team, $11,000 up front for dental work she received in August 2021.

2. The practice sent an insurance claim on her behalf for reimbursement.

3. After not receiving a reimbursement check after six months, Ms. Tucker found out the dental practice sent the claim to the wrong insurance provider and with incorrect information, including the wrong birth year and address.

4. A new claim was then sent to the correct insurer, Ameritas, but was denied because the claim was not submitted within 90 days of service.

5. An appeal filed by the dental practice on Ms. Tucker's behalf was also denied, but Ameritas eventually waived its time limit requirement and promised Ms. Tucker a $1,500 check.
