The Dental Quality Assurance Commission suspended the dental license and moderate sedation permit of a Washington dentist after “charges of unsafe practices and disregard for patient safety when using general anesthesia,” according to a news release from the Washington State Department of Health.

Shuichi Yamaguchi, DDS, of Snohomish County, allegedly failed to provide safe anesthesia, lacked sufficient clinical treatment records for a patient and allowed a dental assistant with an expired credential to assist in a procedure, the Washington State Department of Health release said.

Until the charges are resolved, Dr. Yamaguchi will not be able to practice dentistry in the state. He has 20 days to respond and request a hearing.
