The practice became aware of unusual activity that disrupted access to certain systems in June, according to a notice on the practice’s website. An investigation determined that on or about June 6, an unauthorized actor acquired certain files stored in the practice’s network, some of which contained personal information, including that of former and current patients.

The information involved in the incident varied by individual but may have included names and other identifying information, insurance numbers, dental treatment and medical history information.

Kinsler Family Dentistry reported the incident to the FBI and submitted it to the U.S. Department of Health and Human Services Office for Civil Rights Breach Portal
