Dr. Kelly’s license was revoked May 1. The board’s report cited multiple violations and a “lengthy and extensive disciplinary history.”

The state board barred Dr. Kelly from operating his practice, which has locations in Winston-Salem and Greensboro, N.C. The decision was upheld following the judge’s decision, the report said.
