The practice became aware of unusual activity and launched an investigation, which revealed that on or about Aug. 27, an unauthorized party gained access to and obtained data from the Sango Family Dentistry network, according to an Oct. 24 news release.
Data that may have been impacted includes individuals’ names, Social Security numbers, driver’s license or state identification numbers, dates of birth, medical information, diagnoses, diagnosis codes, treatment information, provider names, health insurance information, billing/claim information, health benefit plan numbers and subscriber member numbers.
At this time, Sango Family Dentistry has not found any evidence that the potentially involved data has been misused, the release said.
