DSOs have continued to experience major growth and innovate the dental industry, providing a new way for dental practices to operate and increasing competition in the field.

Here are 44 DSOs to know in 2025:

Affordable Care (Morrisville, N.C.). Founded in 1975, Affordable Care is a consumer retail healthcare company exclusively focused on tooth replacement solutions, supporting approximately 450 affiliated practices across 43 states. The company works with top dental brands in providing nonclinical support. 

Allied OMS (Southlake, Texas). Founded by four oral surgeons and two sons of physicians in 2020, Allied OMS was created to offer a physician-first alternative to traditional private equity-backed dental groups. The DSO has grown from two initial practices to 32 while focusing on practices in the oral and maxillofacial surgery category.

Apex Dental Partners (Dallas). Founded in 2014, Apex Dental Partners is a DSO with more than 250 providers serving over 200,000 patients annually across more than 40 locations. Apex operates under a framework that prioritizes long-term dentist-patient relationships and clinical independence. With a team of more than 500 professionals, Apex combines the values of traditional private practice with the scalability and support of a modern group organization.

Beacon Oral Specialists (Dallas). Established in 2020, Beacon Oral Specialists has rapidly grown to include more than 90 locations in 11 states. As one of the largest OMS-focused management service organizations in the U.S., Beacon was founded on the principles of collaboration, humility and clinical excellence. 

Benevis (Atlanta). Founded in 2002, Benevis is a dental support organization dedicated to improving access to care for underserved communities across 13 states and the District of Columbia. Through nonclinical services provided to more than 120 affiliated offices, Benevis enables its practices to focus on delivering compassionate, high-quality dental care, particularly to children enrolled in Medicaid and CHIP, and has over 1.2 million patient visits per year.

Cal Dental USA (Los Angeles). Cal Dental USA is a DSO committed to delivering high-quality, accessible care through a patient-centered approach that began in 2015. The organization has expanded to 23 modern dental centers nationwide and was recognized on the Inc. 5000 list for its 123% three-year growth rate.  

Cordental Group (Cincinnati). Cordental Group’s mission is to build on the foundation that dentists have built at its practices and support them to provide the best care to patients while reaching their operational goals. The DSO reached 45 practices earlier this year. 

Dental365 (New Hyde Park, N.Y.). Dental365 is a dentist-led organization dedicated to clinical excellence, educational opportunities and technology advancements. The DSO has zeroed in on salivary diagnostics in recent years, praising the tool as the next standard of care for dentistry. Dental365 supports more than 100 practices across seven states.

Dental Care Alliance (Sarasota, Fla.). Dental Care Alliance is a DSO that partners with approximately 400 allied practices across 24 states, supporting over 900 dentists and 3 million patient visits annually. With a team of more than 5,250 employees, DCA provides non-clinical services including operational, marketing, and compliance support and was recognized by Newsweek as one of America’s Greatest Workplaces for Diversity in 2023. 

EKOS Dental Group (Boynton Beach, Fla.). EKOS Dental Group is a dentist-owned, dentist-led dental organization based in Florida, founded on the principle of creating a true dental home for dentists, staff, and patients. Founded in 2022, it emphasizes its priority of providing comprehensive operational support to empower dentists at every career stage.

Epic4 Specialty Partners (Scottsbluff, Neb.). Epic4 Specialty Partners prides itself on its dedication to quality care and collaboration. The DSO supports specialty practitioners, recognizing the needs of specialists versus general dentists. The dentist-founded DSO supports practices in 15 states.

Gen4 Dental Partners (Tempe, Ariz.). Gen4 Dental Partners closed on a $315 million credit facility last year. In addition to its own growth, the DSO specializes in helping dental practices reach their full potential with its administrative support and operational strategies. 

GPS Dental (Jonesboro, Ark.). GPS Dental supports more than 100 practices with a focus on helping dentists excel clinically without the burden of day-to-day management. GPS is noted for its provider-first approach and operational tools to drive success.

Guardian Dentistry Partners (Miami). Founded in 2018, Guardian Dentistry Partners is a growing dental partnership organization. Since launching initial partnerships in Pennsylvania, Michigan, Texas and Virginia, GDP has expanded to more than 160 practices and 148 partners across 11 states while maintaining its core values and people-first philosophy.

Heartland Dental (Effingham, Ill.). Heartland Dental supports more than 1,700 practices in 38 states. The DSO prioritizes diversity and workplace culture. It was most recently ranked as one of the greatest workplaces in healthcare by Newsweek.

Imagen Dental Partners (Scottsdale, Ariz.). A dental partnership organization designed to empower top-performing dentists through collective ownership, clinical autonomy and centralized business support. Imagen Dental Partners is headquartered in Scottsdale, Ariz. 

Lone Peak Dental Group (Englewood, Colo.). Founded in 2003 by two pediatric dentists with three locations, Lone Peak Dental Group has grown into a pediatric-focused DSO with over 75 offices across 15 states. Lone Peak emphasizes community-based care, often serving underserved areas where access to pediatric dentistry is limited. 

MAX Surgical Specialty Management (Hackensack, N.J.). Formed in 2022, MAX Surgical Specialty Management focuses on surgeon autonomy, independent practice identity and clinical excellence. Founded by Jason Auerbach, DDS, MAX has expanded to represent more than 25 surgeons across 16 locations in New Jersey. 

MB2 Dental (Dallas). MB2 Dental supports more than 750 practices in the U.S. The doctor-led organization emphasizes collaboration and shared success while allowing dentists to retain their clinical autonomy. 

Mortenson Dental Partners (Louisville, Ky.). Mortenson Dental Partners, an employee-owned DSO, operates more than 140 practices in nine states. Through its employee stock ownership plan, every team member has a stake in the organization’s success, encouraging ownership, pride and long-term investment. 

North American Dental Group (Pittsburgh). Founded in 2008, North American Dental Group prioritizes technology, staffing and patient care to support its network. The doctor-led organization aims to encourage a patient-centric approach and collaboration among clinicians. The DSO supports more than 240 locations in 15 states.

OMS360 (Cumming, Ga.). OMS360 is a surgeon-focused oral and maxillofacial surgery support organization that partners with practices to drive growth, enhance profitability and reduce administrative burden. OMS360 offers tailored operational support, recruitment services, performance management and marketing strategies designed to align with each practice’s unique clinical philosophy and patient base. 

P3 Partners (Charleston, W.Va.) P3 Partners is an oral surgery specialty organization supporting practices across the Mid-Atlantic and Southeast regions. Founded in 2017 and Headquartered in Charleston, W.Va., P3 Partners continues to grow its network, currently at 15 dental practices and increasing to 17 by December of 2025. 

Parkview Dental Partners (Sarasota, Fla.). Parkview Dental Partners aims to alleviate the daily stressors for dentists so that they can focus on delivering the best care to patients. The DSO prioritizes clinical education, doctor autonomy and work-life balance. 

PDS Health (Henderson, Nev.). PDS Health is one of the largest DSOs in the U.S., supporting more than 1,000 practices in 24 states. The DSO has spearheaded medical-dental integration, most recently launching PDS Health Technologies, which provides EHR capabilities to external organizations and providers through Epic Community Connect, giving access to a scalable model for EHR integration.

PepperPointe Partnerships (Lexington, Ky.). PepperPointe Partnerships is a doctor-owned DSO that supports 115 locations across Kentucky and Virginia. Unlike traditional models, PepperPointe does not own practices — it supports locally led groups in preserving autonomy, legacy and culture. Offering succession planning, operations and leadership development, it enables clinicians to thrive while maintaining control. 

Positive Impact Dental Alliance (Oklahoma City). Positive Impact Dental Alliance supports 24 practices and is dedicated to empowering private practitioners while fostering a culture of mentorship and growth. Recognized for community involvement, the group hosts Free Dental Days, engages in mission trips and supports underserved populations. 

Qualitas Dental Partners (Boston). Qualitas Dental Partners is a partnership organization serving a full spectrum of dental specialties and is headquartered in southern New England. It embodies its partners’ priorities through a shared mission to deliver exceptional care to patients.

Reveal Dental (Cedar Park, Texas). Reveal Dental provides comprehensive, patient-centered care with a focus on comfort and personalized treatment planning with advanced diagnostics. Reveal Dental is committed to redefining the dental experience for individuals and families in the Cedar Park community.

Rock Dental Brands (Little Rock, Ark.). Rock Dental Brands collaborates with dentists to identify pain points for practices and tailor plans for each partner. The DSO removes the administrative burden to allow dentists to prioritize patient care, without taking away their autonomy. The company supports more than 100 offices in the U.S.

Rodeo Dental & Orthodontics (Irving, Texas). Rodeo Dental’s mission is to expand care for patients from underserved communities and increase accessibility through education, affordable prices, streamlined services and increased availability. The company supports more than 40 offices in Colorado, Arizona and Texas.

​​Sage Dental (Boca Raton, Fla.). With more than 65 locations, Sage Dental delivers community-focused dental care with general dentistry, specialty services and hygiene. Sage provides pediatric services and orthodontics as well as cosmetic procedures and oral surgery. 

SALT Dental Partners (Phoenix). SALT Dental Partners is a doctor-led, multispecialty DSO with more than 150 practices nationwide. SALT has achieved more than 300% growth in two years and expanded into 20 states. Known for its emphasis on autonomy and scalable infrastructure, SALT empowers clinicians to lead with clinical freedom while benefiting from centralized resources. 

Smile Brands (Irvine, Calif.). Smile Brands boasts more than 600 practices in the U.S. In addition to prioritizing patient care, the DSO gives back to its communities through its Smiles for Everyone Foundation, which provides free care to communities worldwide with limited access to dental care. The company has also been honored for its workplace culture, being recognized as one of the best places to work by Glassdoor. 

Smile Doctors (Dallas). Smile Doctors is the largest orthodontics-focused DSO in the U.S., with more than 450 practices. The DSO provides a variety of treatments to enhance patients’ smiles. The company was recently named one of America’s Greatest Workplaces in Health Care for 2025 by Newsweek.

Smile Partners (Troy, Mich.). Smile Partners offers administrative support to private practices and boutique group practices throughout the Midwest, Southeast and Western regions of the U.S. The DSO supports more than 100 practices in Michigan, Georgia, Illinois, Colorado and Alabama. 

Specialized Dental Partners (Franklin, Tenn.). Specialized Dental Partners is a growing dental support organization focused exclusively on specialty care, partnering with leading endodontists, periodontists and oral surgeons nationwide. The organization, founded in 2018, has helped many practices achieve double-digit growth in endodontics, periodontics, and oral and maxillofacial surgery.

SPP Dental Partners (Salt Lake City). SPP Dental Partners is a dentist-founded and dentist-led dental partnership organization dedicated to preserving private practice autonomy while providing comprehensive support services. Established by three dentists in 2020, SPP’s network spans across Utah, Idaho, Nevada, California and Washington. 

Straine Dental Management (Sacramento, Calif.). Straine Dental Management is one of the fastest-growing DSOs in the U.S. The DSO has expanded its network in several states this year, including two new states. Straine Dental Management supports practices across 14 states.

The Aspen Group (Chicago). From its beginnings over 20 years ago with Aspen Dental, TAG, The Aspen Group, has grown into the retail healthcare support we see today, now partnering with over 1,300 offices and 20,000 professionals across 46 states. Headquartered in Chicago, TAG empowers providers across multiple fields through its family of brands. 

The Smilist (Great Neck, N.Y.). The Smilist supports a network of orthodontists throughout the Northeast region of the U.S. The DSO recently landed a $285 million unitranche debt facility to fuel its growth, which helped push the company past 100 offices. The organization also values a supportive workplace culture, and is a certified Great Place to Work. 

T Management (Atlanta). T Management, founded in 1999, serves the Southeast, with a strong presence in the Atlanta area. The company offers comprehensive operational support including staffing, payroll, daily finances, and general practice management. With a dedicated team experienced in the day-to-day realities of dental operations, T Management helps practices grow by streamlining business functions. It currently has 12 dental practices. 

URBN Dental (Houston). Founded by cosmetic dentists in 2016, URBN Dental delivers aesthetic-driven, high-quality dental care. The practice is committed to ethical dentistry by exclusively using non-toxic, BPA-free materials sourced from leading U.S. laboratories and is dedicated to its patient safety, environmental responsibility and clinical excellence.

U.S. Oral Surgery Management (Irving, Texas). Founded in 2017 with the mission to support oral and maxillofacial surgeons through a unique partnership model, U.S. Oral Surgery Management offers non-clinical expertise in operations and growth strategy while preserving clinical autonomy. USOSM partners with 122 dental practices.
