North Carolina dentist's license revoked

The North Carolina dental board revoked the license of a dentist after an investigation into several alleged issues, CBS affiliate WFMY News 2 reported May 2. 

Andrew Kelly, DDS, operates the Dental Center of the Carolinas, which has offices in Winston-Salem and Greensboro, N.C. 

The North Carolina State Board of Dental Examiners accused Dr. Kelly of lying to patients, practicing sedation without an active permit, malpractice, and having incomplete patient records and inaccurate drug logs.

The dental board revoked Dr. Kelly's license May 1, citing a series of violations and a "lengthy and extensive disciplinary history," according to the board's report.
