The Washington Dental Quality Assurance Commission indefinitely suspended the license of Sandy Montano, DDS.
Dr. Montano violated multiple sanitation and infection control requirements in November 2021, according to a May 20 news release from the Washington State Department of Health.
In 2022, Dr. Montano had his license suspended after refusing to allow an investigator from the department of health to conduct an inspection.
Dr. Montano must complete a mental and physical examination before petitioning for reinstatement, the release said.
