1. The Washington Dental Quality Assurance Commission denied the dental license of a dentist in Holt, Mich.

2. Jeff Fordice, DDS, of Fairmont, Minn., is set to retire after more than 40 years in the dental industry.

4. The Arizona Board of Dental Examiners revoked the license of a dentist in Flagstaff, Ariz., who used pliers on a patient during surgery.
